What is a Person-Centered Plan?
A Person-Centered Plan is a plan you create that includes information all about YOU and your life goals. This Plan helps support your ability to achieve the life you want to lead by respecting you as an individual, assisting you in gaining independence, and building upon your strengths.
Person-centered planning takes a team approach. This team may include family, friends, and other supporters who are important to you. You are the one who decides who will participate as supporters in developing your Person-Centered Plan.
A Person-Centered Plan is a living document developed through a process designed to empower you to express your self-determination and practice your self-advocacy, as you plan for your future. It provides a means by which your supporters and the systems involved in your life learn more about you, from your perspective, as well as from the perspectives of a variety of people who know you best.

Diverse Ability Incorporated values the power of peer support and creates thoughtful matches that consider both your unique needs and the lived expertise of the Peer-Based, Person-Centered Planning Facilitator who is assigned to guide your Peer-Based, Person-Centered Planning Services process.
Your assigned Peer-Based, Person-Centered Planning Facilitator will have experience in navigating some of the opportunities and challenges you are navigating, and we believe this provides a strong foundation upon which to build your individualized plan.
The key component of person-centered planning is Discovery, which is a process that explores the strengths, skills, likes, dislikes, hopes, dreams, opportunities, obstacles, doubts, and concerns that you and your chosen support team identify. Discovery is a critical and comprehensive process that considers the life of a person across systems, environments, and domains, and embracing this process creates an opportunity for you to gain clarity, establish priorities, and set a course for your future.
The information captured during discovery is gathered, synthesized, and shared with you and members of your chosen support team as you work together to develop your Action Plan.
The components of the action plan include:
- Meaningful goals and action steps which you approve of
- Identification of the resources and supports available and/or that you will use to accomplish your planned activities
- Documentation of the timelines associated with your action plan
- Assignment of responsibilities for implementation of action plan activities
It is important to note that while all discovery is documented in the planning document, you are the final decision-maker for the actual action plan.

A Peer-Based Services Professional will contact you to gather some preliminary information, answer any questions you may have, and schedule a meeting to start your Discovery process.
Early in the process, you will be asked to provide Diverse Ability Incorporated with a list of the people you would like to participate in your Peer-Based, Person-Centered Planning process, along with their contact information. Those who agree to participate become part of your team and will be asked to complete a questionnaire designed to capture more about you from their perspectives.
Once you and your support team have completed the Discovery process, our Peer-Based, Person-Centered Planning Facilitator will synthesize all the information to present to you and your team. This information will help to guide your goal-setting process, which will result in the development of an Action Plan, which will happen during a meeting (or series of meetings).
On average, completing a Peer-Based, Person-Centered Planning process takes about 10-15 hours from start to finish, and typically occurs over a 4-6 week time period. Each individual and planning team is unique, and the timeline may vary.
Peer-Based, Person-Centered Plans may be completed virtually, by phone, in-person, through online questionnaires and meetings, or through a combination of these. Accommodations and supports will be provided to you and members of your chosen support team in order to ensure success.
Review and follow-up are built into the timelines of your Action Plan so that you and your team understand expectations going forward.

- The fee for an initial Peer-Based Person-Centered Plan is $750.00.
- The fee a follow-up Peer-Based Person-Centered Plans is $375.00
- The fee for developing a Person-Centered Portfolio, which includes Discovery and not Action Plan development is $500.00.
- The fee for an urgent Peer-Based Person-Centered Plan is $1,000.00.
At present, through partnerships and diversified funding sources, we are able to provide this service at no cost to most students, youth and young adults. However, we look at each Peer-Based, Person-Centered Planning Service Request on an individual basis.
