DESCRIPTION:The Transition Institute is free for transition-aged youth and young adults (14- 26 years old) with disabilities and their parents. \nTo learn more and register\, visit the Encircle Families Transition Institute 2025 registration webpage. \nPARENT TRACK:  \n\nLearn about transition and employment services provided by the Division of Developmental Disabilities (DDD) and Vocational Rehabilitation and how they work together. Additionally\, parents will learn about Social Security\, ABLE Accounts\, Supported Decision-Making\, transportation options and voting rights. There will be an opportunity for those eligible to register to vote. The day will conclude with a presentation called Imagining a Great Life!\n\nYOUTH TRACK: \n\nCentered on YOUth – Youth and young adults are invited to join Arizona Young Adult Leaders from Diverse Ability Incorporated for an overview of self-determination and self-advocacy and an opportunity to recognize and appreciate their own skills that lead to greater independence. Learn about Person-Centered Plans and how they consider your strengths\, talents\, and gifts. PCPs allow youth to choose activities\, make decisions\, and set goals based on their interests and preferences. Participants will actively engage in a piece of the Person-Centered Planning process\, called Discovery.

DESCRIPTION:The African American Conference on Disabilities (AACD)\, co-founded by Renaldo Fowler and David Carey\, is the only comprehensive conference that addresses disability rights issues at the intersection of race and disability. The conference examines avenues for self-advocacy to ensure access to necessary services and enforcement of civil rights in the African American and disability communities. \nThe AACD is designed to provide education and knowledge to be proactive\, not reactive and for the African-American community to obtain information so it will empower them and the community on how to better serve the community and themselves.  Additionally\, it is a platform for policymakers and providers to come and hear directly from the African American community.

DESCRIPTION:The Center on Youth Voice\, Youth Choice is a resource center for youth who have disabilities. They talk about how you can have control over your decisions after you become a legal adult. One of the ways you can do this is by using alternatives to guardianship (A2G). \nThe Center on Youth Voice\, Youth Choice does research\, advocacy\, and education about alternatives to guardianship. They also support youth how have disabilities by talking about alternatives to guardianship and working together. \nHear from Center on Youth Voice\, Youth Choice’s Arizona Youth Ambassadors about the great work they are doing around supported decision-making and other alternatives to guardianship in their state.
